The applications are available in Word format below. Where possible, please complete them in Microsoft Word and retain a copy for your records and future use.

For renewal you will need to complete the full application process.


5 or more employees, Designers and CDM-C's:

Appendix 2

This application is for completion by suppliers who employ 5 or more people. It is also to be completed by Designers or CDM-Cs. It is important you answer all relevant questions with as much detail as possible and send only the supporting documentation and evidence required for this assessment.

Fewer than 5 employees:

Appendix 2A

This application is for completion by sole traders and suppliers who employ fewer than 5. (It is not to be completed by Designers or CDM-C's)

Care Service Providers:

Appendix 2B

This application is for completion by day care, domiciliary, residential and independent living providers. It is important you answer all relevant questions with as much detail as possible and send only the supporting documentation and evidence required for this assessment.

Group or other Scheme:

If you are applying as a group or through another scheme please contact the CHAS administration team for futher details and an application form. The minimum number of offices that can be registered as a group application is three and they must all use the same safety management system.


CHAS Assessment:

Once you have completed your application, apply to CHAS for an assessment by contacting the administrator for the name and address of the next available assessor.
You must tell us if you are applying as a Designer, CDM-C or Care Service Provider.


CHAS Accreditation:

If you have achieved compliance in the last 9 months, you can apply for accreditation. If you wish to renew your accreditation, you will need to reapply for a full-assessment and an accreditation pack on a yearly basis.
