Follow the instructions below according to which Browser you are using.

Download the CHAS Questionnaire

The CHAS Questionnaire is available in Word format.
It is highly recommended that where possible, you should complete the selected questionnaire using Microsoft Word and retain a copy for your records and future use. If you wish to apply to CHAS for an assessment you must contact the CHAS administrator before submitting your application.
5 or More Employees

This questionnaire is for completion by applicants who employ 5 or more employees. It is important you answer all the questions with as much detail as possible and send all the supporting documentation required for this assessment

Download the Designer Pre-qualification Questionnaire

This questionnaire is to be completed by Consultancies who wish to pre-qualify as designers. For CHAS prequalification purposes we limit the definition of a designer to a person or practice involved in architecture, civil or structural engineering or building survey. It is important you answer all questions providing as much detail as possible then send it with supporting documentation to tim.gough@kingston.ac.uk.

Download the CDMC Joint Questionnaire Assessment standards

This questionnaire is to be completed by individuals or practices who wish to pre-qualify as a CDMC. It is important you answer all the relevant questions and provide as much detail as possible.
Once you have completed it please contact us via the web site and let us know your CDMC application is ready (Stress it is a CDMC application). We will reply using e-mail to tell you the name of the assessor who will undertake your assessment. This person is a practising CDMC safety practitioner and trained to our standards. They will complete their assessment within 15 working days.
